What is Gynecomastia?
Gynecomastia is a condition characterized by the enlargement of breast tissue in males, caused by an imbalance of hormones, specifically an increase in estrogen levels relative to testosterone. It can affect one or both breasts, and can cause pain or tenderness in the affected area. Gynecomastia is estimated to affect approximately 32% to 65% of adolescent males and up to 70% of men over the age of 50. It’s important to note that while gynecomastia is not usually a serious medical condition, it can cause emotional distress and affect a man’s quality of life.

Gland, Fat, or Both?
Male chest enlargement is commonly classified as either true gynecomastia or pseudogynecomastia, and the distinction matters because it changes the operation.
True gynecomastia is the proliferation of glandular breast tissue, typically presenting as firm, rubbery tissue beneath the nipple and areola, and generally driven by an imbalance between estrogen and testosterone. Because the enlargement is gland rather than fat, removing fat alone will not correct it — the gland itself has to be addressed. That is what this procedure does.
Pseudogynecomastia, sometimes called lipomastia, is not glandular at all. It is an accumulation of excess fat in the chest area. Where that is the whole picture, gland removal is unnecessary and chest liposuction is the more direct procedure. Many men present with a mix of the two, and the proportion is what we assess at consultation. What Results Look Like, and When
Patients notice an immediate improvement, but the optimal result appears over the following three to six months as swelling resolves and the skin settles. Bruising and swelling usually subside within about a week, and most men return to normal activity after a day or two.
The compression garment does a great deal of the work in that period and should be worn at all times except when bathing. High-impact exercise such as running, and the resistance load in weight training, both need to come down for a few weeks before building back up. Following the post-operative guidance closely has more influence on the final contour than most people expect. Individual results vary.
Can Gynecomastia Come Back?
Gynecomastia reduction at Imagen is more than fat removal. During the procedure the surgeon also removes a portion of the mammary gland, and that gland is usually the main reason for the breast-like appearance in the first place. Removing it is what makes the change hold.
Results are long-lasting when paired with a healthy diet and stable lifestyle. It is worth knowing that a portion of the gland remains after surgery, and remaining gland tissue can still respond to the same hormonal triggers that caused the original enlargement. That is a conversation worth having at consultation rather than after.
