The Two Kinds of Belly Fat — and Only One Can Be Treated
Not all stomach fat behaves the same way, and the difference decides whether liposuction can help you at all. The abdomen holds two separate layers. Intra-abdominal fat sits deep, behind the abdominal muscles and around the organs. Extra-abdominal fat is the superficial layer between the skin and those muscles — the fat you can actually pinch.
Liposuction reaches the superficial layer only. Deep intra-abdominal fat is not amenable to cosmetic treatment or surgery; frustrating as that is, it comes down with diet and exercise or it does not come down at all. It is why two people with similar waistlines can leave a consultation with completely different answers. The question is never how large the stomach is, but which layer the fat is sitting in.

Why Loose Skin Changes the Recommendation
How much fat is present is only half the assessment. How loose the skin is — and whether stretch marks are already there — often decides which procedure actually makes sense. That matters more on the abdomen than almost anywhere else, because after pregnancy or significant weight loss the skin, not the fat, is frequently the real complaint.
Non-invasive fat reduction does nothing for the skin. It can reduce a small, discrete pocket of fat, but it leaves elasticity exactly where it found it. SmartLipo behaves differently: because the laser treats the skin from underneath, it stimulates collagen production, and that produces skin retraction as the area heals. You are treating the fat and the envelope around it in the same session. Where more tightening is wanted, Renuvion is added. There is a limit to this, and it is worth saying plainly: if there is genuinely too much loose skin, no amount of fat removal will correct it, and a skin-removal procedure is the more honest recommendation. Better to hear that at a consultation than after a procedure.
What SAFR Lipo 360 Covers
Most patients who ask about stomach liposuction are not describing the stomach in isolation. That is why Imagen most often treats the abdomen as part of SAFR Lipo 360 — upper abdomen, lower abdomen and love handles in a single session, with skin tightening included.

The First Week of Recovery
The compression garment stays on the first night, which means no shower. Because we do not use drain tubes, the incision sites leak — sometimes a lot — and that drainage is generally finished within the first 24 hours. Old towels, incontinence pads on the furniture and a shower curtain liner under the sheet will spare your mattress.
Then get up and move. We ask patients to take a short tour of the house every couple of hours during waking hours; movement mobilises fluid, encourages drainage and helps prevent blood clots. Eat protein because your body needs it to heal, keep salt down because salt encourages fluid retention and increases swelling, and drink plenty of water to flush out the fluid used to numb the area.
You come back to our Omaha clinic the following morning. We remove the garment and dressings, one of our surgeons checks that everything is healing as it should, and you go into a fresh garment. You will want to look. What you see then is emphatically not your result: at that point there is a great deal of swelling, fluid, bruising and loose skin. The day after surgery is usually the most uncomfortable one, in much the same way that the day after training too hard is the sore one.
Most patients plan on three to five days away from work and manage light desk work from home in that window. By the end of the first week most notice a distinct turn — swelling is around its peak, but the discomfort has clearly eased. Any stitches come out at roughly a week. Numbness across the treated skin is expected and temporary, and firm lumps and bumps at this stage are swelling rather than your final contour. If one area feels markedly more tender than the rest, or a lump feels soft and shifts when you press on it, call us rather than waiting for your next appointment.
Weeks Two and Three
You will still be swollen at two weeks, though any bruising should be fading. Itching is common, and so are strange sensations — tingling, brief zinging pains — as the nerves in the treated skin wake up. The garment stays on around the clock unless we tell you otherwise.
By week three the compression garment has usually become both your best friend and your worst enemy. Swelling continues to improve, but firm areas remain. On the abdomen specifically, swelling lingers longest in the dependent areas, where gravity pulls fluid downward: the lower tummy, the area just above the belly button, and the lower back. If the garment is creasing the skin, foam placed inside it with a snug camisole underneath solves it.
After three weeks most patients are healed enough to return to a normal exercise routine, wearing compression during any high-impact workout for the extra support. Keeping Your Result
Fat cells removed during liposuction do not grow back, which is what makes the change a durable one. The cells left behind can still enlarge, though, so the result tracks your habits from here. Patients who already had an exercise routine do best when they simply keep showing up — even thirty minutes of walking in the early weeks helps the swelling and keeps the habit intact, which would be a poor thing to lose after the investment you have just made in yourself.
