Why Back Fat Is So Hard to Train Away

Genetics decide where your body stores fat, and some people are simply predisposed to store it across the back. If extra weight has always settled on the upper back in your family, it will likely settle there for you too, whatever your diet and training look like.

The layer involved is subcutaneous fat — the pinchable layer that sits between the skin and the back muscles. That matters, because it explains a frustration we hear constantly: strengthening the muscles underneath does not remove the layer sitting on top of them. You can build a strong back and still not see it.

Hormones shift the picture as well. Elevated cortisol encourages the body to hold fat, and the change in estrogen through menopause moves fat to new places. That is why so many women notice fullness along the bra line for the first time in their forties or fifties.

Where Back Fat Collects

Back fat is rarely spread evenly. It tends to gather in three places: across the upper back between the shoulders, along the bra line where a band creates a visible roll, and lower down as rolls above the waist.

Each behaves a little differently, and a plan that treats the back as one flat surface tends to leave the transitions looking abrupt. Treating the bra line without addressing what sits above and below it is the most common reason a result looks unfinished. Fullness at the bra line also frequently continues into the underarm, which is why back and arm liposuction are often planned together.

Why the Back Behaves Differently in Surgery

Back fat is denser and more fibrous than the softer fat on the abdomen or flanks, and the skin covering it is thicker. It does not release as readily, which is exactly why some patients are told their back fat cannot be treated after a disappointing result elsewhere.

It can be treated. It requires a technique suited to fibrous tissue and a patient, even hand across a wide surface, because unevenness on a broad flat area shows more than it would on a curved one.

How Imagen Treats the Back

Treatment starts with the tumescent technique. A saline-based solution is infused into the fat before anything is removed. It swells the fat so it releases more easily, limits bleeding, and numbs the area during the procedure and for a while afterwards. This is what makes it possible to treat the back with the patient awake, under local anesthesia, going home the same day.

Recovery

A compression garment is worn for several weeks. It controls swelling and helps the skin settle against the new contour, and on the back it does a second job: it keeps steady pressure on a broad area that is awkward to support any other way.

Most patients are back to desk work within a few days. Strenuous exercise, and anything that loads the back in particular, waits considerably longer — your surgeon will tell you when.

Swelling hides the result at first. The back is a large treatment area, so the true shape appears gradually over months rather than weeks. Individual results vary.

Keeping Your Result

Fat cells removed from the back do not come back, but the ones left behind can still enlarge, so a stable weight is what protects the contour long term.

Strength work pays off differently after treatment. Rowing and pulling movements build the muscle directly under the area that was thinned, and with the covering layer reduced that definition is finally visible. Am I a good candidate for back liposuction?

An ideal candidate for fat removal through back liposuction is someone who is in good health, who hasn’t been able to get rid of undesired stubborn fat deposits with diet and exercise. You should be at or close to your ideal weight with good skin elasticity and realistic expectations for the results of your cosmetic procedure. If you have a significant amount of skin laxity, you may get a better result from a surgical procedure. A back lift or a circumferential body lift is similar to a tummy tuck.

Can liposuction get rid of back fat?

Back liposuction surgery can assist you to reduce unsightly bra rolls (back boobs) and the infamous Buffalo hump in the upper back to give you smooth and beautiful curves. How to prepare for back fat liposuction?

For two weeks before your planned surgery, avoid all blood-thinning medications, alcohol, and smoking; these substances affect bleeding and recovery.

Be sure to discuss any medications that you take on a regular basis with the team, you may need to stop some of them before your procedure.

You will be given oral sedation and it is not safe for you to drive after your back lipo. Plan for a ride to and from your procedure.

Wear loose clothing to your procedure to make changing more manageable and more comfortable. Be sure it’s clothing you don’t mind getting soiled (with tumescent liposuction you will have some drainage from your incisions the first day).

How long is the recovery from back liposuction?

Most of our patients are back to a pretty normal life 2-3 days after their back lipo. You will have some post-op discomfort at that point, but it’s more like soreness or an ache rather than pain. You will need to wear a compression garment for 4 weeks after the back liposuction procedure. You can go back to some exercise right away, but will want to avoid high impact exercises for 3 weeks.

How long do back liposuction results last?

Liposuction is a permanent fat reduction procedure, but it is not a weight loss procedure. Your goal should almost always be either maintaining a consistent weight or losing weight. If you eat more calories than you burn, you body will find a storage spot for them.

Are there alternatives to back liposuction?

Depending on the distribution of your fat and the quality of your skin, you may be a candidate for alternatives to back liposuction. If you have only small areas of fatty tissue (for example the bra bulge) then CoolSculpting may be a viable option. If you have excessive skin laxity, you may get a better result with plastic surgery and a skin resection procedure like a back lift.
