Frequently Asked Questions About Fractional Laser Resurfacing

Fractional laser therapy works by using beams of light energy to remove columns of skin while leaving the surrounding tissues intact. The tiny wounds that are created by the laser stimulate a healing response, activating collagen production and bringing growth factors to the skin. The intact areas of epidermis allow a faster healing time. Generally, the fractional lasers treat about 20% of the surface of the skin with these micro-columns. Healing from a fractional laser therapy takes about 1-2 weeks, with five to seven days of “social downtime.” This is compared to a fully ablative laser therapy which can require several weeks of social downtime. A single fractional therapy will give you impressive results. You will notice a change in as little as a week with continued improvement for up to 12 weeks after the therapy.

Since lasers are attracted to pigment in the skin, Imagen uses two types of laser technology so both light and darker skinned patients have safe treatment options for Fractional treatment. The two fractional lasers we use are CO2RE and Pearl [Erbium]. The Erbium laser energy is safer for darker skin types but will require preparation of the skin with topical products to help prevent hyperpigmentation (a skin discoloration) in response to the treatment.

At Imagen, we offer all Fractional patients oral sedation to ensure their comfort throughout the laser treatment. We use a topical numbing cream followed by nerve blocks and local anesthetic injection as well. Following the procedure, you can expect a sunburn-like sensation with redness and flaking; some experience mild swelling depending on the treated areas (with treatment of the eyelids the swelling can be significant).

Imagen preps all Fractional patients with medicine so they are comfortable throughout their treatment. Following the procedure, you can expect a sunburn-like sensation with redness and flaking. All patients will experience the side effects of redness and swelling, many will have some bruising. The bruising is typically a result of the anesthetic rather than the laser itself. Some patients will notice a prolonged pink tone to their skin as a result of the cosmetic procedure. This typically resolves over several weeks to months.

It is very important to follow all post treatment care instructions. If you pick, rub or scrub off the outer layers of skin, you will get irritation and inflammation which can lead to areas of hyperpigmentation. This risk is particularly high in darker skin types. Some skin types may develop hyperpigmentation in response to laser treatment even if you follow all of the post op instructions. Pretreatment with a Hydroquinone Bleaching Cream and Retinoic Acid Cream can help to prevent this complication.

If a treatment is too aggressive, it is possible to have burns on the skin. The experience of your physician is key in avoiding complications, especially in darker skin types. Darker skin types are also more prone to complications involving hypopigmentation, a loss of pigment in the skin, which is more difficult to treat than hyperpigmentation.

You will need eye protection throughout the procedure. The best type are eye shields that are worn inside of your eyelids (like contact lenses). This allows treatment of the eyelids themselves and does a better job of protecting the eyes.

If you have a history of cold sores or oral herpes, be sure to let your physician know. You will want to take an antiviral around the procedure to help prevent an outbreak of the lesions.

Fractional patients will see results from just one treatment. Initial changes will begin to appear within one week and will continue to develop over the coming months. That being said, the number of treatments you will need will depend on the conditions that you are treating and their severity. A patient in their early 30’s treating minimal signs of aging will likely have a desirable result with a single treatment. Patients who are treating deep scarring or advanced aging may benefit from a staged treatment plan including repeated laser treatments combined with prescribed skin care products and other rejuvenation and skin treatments.

The healing process of CO2 laser resurfacing requires patience. Immediately after your laser skin resurfacing treatment you will notice a heat in your skin. Most patients liken this to the sensation you get with a bad sun burn. Your skin will be very red and you may have some pinpoint bleeding. You will have extensive swelling related both to the numbing and the laser itself. The day of your treatment, you will want to keep the skin moist with aquaphor or vaseline to prevent the skin from drying out. You will do vinegar soaks every few hours, which helps prevent infection in addition to cooling the sensation in the skin. We recommend that you store your vinegar solution in the refrigerator so that it feels refreshing on the skin. The heat typically dissipates over the first 24-48 hours. The redness will improve daily over the next 4-5 days and by a week you should be left with a pink tone to your skin. This should be pretty easily covered with makeup (we love the Bye Bye Red concealer). The swelling generally persists for just a few days and then also improves significantly. You may notice the dot pattern from the laser on your skin for several weeks after the treatment, but it is also easy to conceal. It is imperative that you avoid sun exposure after your treatment. After several days you can apply sunscreen but you should also use a hat or shade as the new skin is at higher risk for sunburn and sun damage.

There are several factors that contribute to the common complaint of itching after a fractional treatment. First, similar to the itching you get after a bad sunburn, the skin releases histamines in response to the injury. Because of the histamine response, oral antihistamines (such as Benadryl) can also be used, but use caution if you are taking sedating pain medications. Do not apply topical antihistamines to the treated area. The cooling of the vinegar soaks can provide you with some relief from the itching. We often recommend that patients soak a soft washcloth in the vinegar solution and then freeze it. They can then apply the frozen solution to the skin and as it melts it will cool and relieve the discomfort. If the itching is severe, your doctor may recommend a topical steroid ointment.

Once the majority of the redness and swelling resolves (after 3-5 days) you will begin to appreciate the improvements in the treatment area resulting from your fractional procedure. Patients will notice a smoother skin texture with less pigment concerns as well as an improvement in fine lines. As your skin begins to produce new collagen, you will appreciate an improvement in the deeper lines as well as a tightening of the skin. These results typically take about 3 months to fully develop.

The results of your laser skin resurfacing will typically last between 1 and 5 years. The wide range is a reflection of the wide range of patients and concerns that we treat with fractional lasers. If you are treating acne scars for example, the results will be long lasting as long as the acne is under control. If you are treating sun damage and you have continued sun exposure, your results won’t last very long. If you use SPF regularly, you may extend your results. In regards to using laser resurfacing for treatment and prevention of aging and rejuvenation, the duration of your results will depend on how old you are, your skin care regimen, what other skin treatments you are having, how fast your body breaks down collagen and many other factors.

Fractional resurfacing at Imagen starts at $750 for single-area treatment. A single area may include the area around the eyes or the smokers lines around the mouth. Full Face treatment is $1,750. The most commonly treated areas are the full-face and neck, though some choose to target the eyes or specific spots and scars.

By building collagen, you can get an improvement in fine lines and wrinkles as well as acne scars. We love to pair our laser genesis with PRP for treatment of acne scarring. The increased collagen also helps to diminish large pores.

The laser is a near-infrared (NIR) Nd:YAG laser with a wavelength of 1064-nm. This particular wavelength has an affinity for reds and as such is frequently used to treat vascular or capillary concerns such as rosacea and other conditions with diffuse redness. For these patients we typically combine regular laser treatments (every 1-3 months) with iS Clinical Pro Heal Serum, which helps control the inflammation associated with rosacea. Targeting the redness also helps with early acne scars.

The laser helps to reverse the processes associated with aging and can improve sun damage, uneven skin texture and pigment concerns such as age spots and discoloration of acne scars.

The heat of the laser can be used to break up dirt and oil, which can lead to acne and black heads. The laser can be used when active acne is present and may even help to improve the breakouts.

Laser Genesis treatment can be used on any part of the body. We most commonly use it on the face, but you can also treat the neck and décolleté, which are commonly ignored areas. We also combine laser genesis with laser vaginal rejuvenation to help improve the tone and texture of the skin of the labia.

As with all aesthetic treatments, results will vary. For most patients and most conditions, a single laser genesis treatment isn’t going to result in amazing outcomes. The exception to that would be as a short lasting plumping treatment prior to an event.

For patients who have regular treatments, laser genesis can have beautiful results.

The answer to this question depends on what your goals are with the treatment. If you are looking for improvement in redness or issues with blood vessels, those results are usually faster than results for treatment of scars or trying to improve the appearance of fine lines. Because the improvement of these skin conditions depends on new collagen production, the optimal results will typically take several months.

This also depends on your goals. Very few patients will have amazing results with a single treatment. Most of our scar treatment plans include 6-10 treatments, while our anti-aging treatment plans may have 4-6 per year for maintenance.

For most patients first starting a course, we will recommend treatment every 2-4 weeks for the first few treatments. After that, we may spread them out to monthly for scar treatments or every 2-3 months for anti-aging.

If you are treating scars and you don’t have a recurrence of the cause of the scars, the treatment will be long lasting and shouldn’t need maintenance. If you are treating the signs of aging, you should plan on a maintenance treatment plan. If you stop treatment, the results will likely last for 6-12 months. A clinical skin care regimen and daily sunscreen use will help extend the duration of your results.

The first thing that will happen when you come in for your first treatment will be some paperwork. Once that is complete, we will have you wash your face (you want it completely clean for a treatment, not even any creams or serums) and take photos. Once that’s complete, we’ll have you lie down and relax. We will place eye shields over your eyelids to protect your eyes. They look similar to tanning goggles. We typically start the treatment on the forehead and then move around your face. The treatment feels warm, kind of like the rush of hot air that comes out of an oven when you open the door, but most patients feel no discomfort, some even say it’s relaxing (like lying in the sun). There is no anesthetic needed. The treatment takes about 45 minutes to an hour to complete. After the treatment, you may notice some diffuse redness from the heat of the laser (like if you had been outside in the heat). This typically resolves in the next half hour as your skin cools. You can go back to all normal activities as soon as the laser is complete. You can also go back to your normal skin care regimen as well as apply makeup if you would like. We will schedule your follow up appointment for your next treatment before you leave the office.

One thing to note, for the gentlemen who have facial hair the treatment can be uncomfortable. We recommend that you shave the morning of your treatment to make sure that there is as little hair as possible in the area. Alternatively, we can also avoid the area with facial hair if you don’t want to shave.

There are few side effects from laser genesis. The biggest risk of the laser is actually to your teeth. If the heat is allowed to concentrate on your enamel, it can crack your tooth. For this reason, we will ask that you keep your lips closed and not talk during the treatment. As previously mentioned, you will likely have some mild pinkness to your skin.

There are a lot of options for treatments when it comes to anti aging and scar treatments. They all have risks and benefits and the optimal treatment for you will depend on your individual case.

Both treatments will stimulate collagen production. Microneedling will have some recovery time of 1-2 days of social downtime, while laser genesis has no downtime. Laser genesis will be better at treating red conditions and acne and microneedling when combined with injected PRP may be better at smoothing uneven skin texture.

A chemical peel is a treatment that will cause sloughing of the skin, usually resulting in 2-7 days of downtime depending on the depth of the peel. Similar to microneedling, this will typically be a better treatment for skin texture and a faster treatment for pigment concerns.

There are other treatment options for laser hair reduction, but caution must be used with things like Intense Pulsed Light (IPL) as complications are common in skin with more melanin.

The laser hand-piece cools and soothes as it slides along your skin in the treatment area. When the pulsed beam of light is applied, some patients experience a mild pinching or stinging sensation. No local anesthesia or pain medication is required. We typically numb the skin with ice before treatment begins, which helps to reduce discomfort during the treatment.

Because the laser focuses on pigments, the ideal candidate for hair reduction treatment is a patient with light skin tone and dark hair. The obvious difference between skin tone and hair color make it easy to treat the dark pigments without affecting the light skin. If you have darker skin types, that doesn’t mean you’re not a good candidate, it just means that it may take more treatments to achieve your desired result. Patients with very light hair or white/grey hair will likely not see much benefit from laser hair removal.

Although patients encounter few side effects, one of the most commonly noticed are small reddening and local swelling of the skin, just like a sunburn. These effects usually only last for a few hours or less.

Most areas call for 5-8 treatments. However, the number of treatments needed for ideal long-term benefits relies on several factors – hair thickness, the treatment area and your hair growth cycle. Interestingly, not all hairs are actively growing simultaneously and actually undergo three distinct phases: growth, regression, and resting. Therapies work by disabling the hair that is primarily in the active growth phase during the time of treatment. Since hairs will enter this active growth phase at varied times, additional treatments are necessary to disable all the hair follicles in a given area. While laser hair removal treatments are long lasting, you will not likely have permanent results. Many of our hair reduction patients return for a “touch up” treatment once a year.

Laser hair removal really has very little recovery time. Promptly following your session, the treated area may appear a bit red and swollen. Three to seven days after the treatment you may encounter what seems to be regrowth of the body hair. In fact, most of these hairs are being shed as a result of your treatment and are not new hair growth. Between treatments you should not wax or pluck your hair, but it is acceptable to shave.

Yes! Hyperpigmentation in the areas of frequent shaving or waxing, especially in patients with melanated skin, is very common. There are several different treatment options for this concern, including topical bleaching creams, chemical peels and laser skin resurfacing.
